What is Support at Home?
Support at Home (SaH) is a government-funded programme designed to provide older Australians with the necessary support and assistance to help them remain living independently in their own homes for as long as possible. It offers funding for tailored home help services that can include a range of supports. In partnership with Trilogy Care, Neighbourhood Care offers a wide range of in-home aged care services, including help with personal care, household tasks, nursing, allied health and staying connected to your community. All support is tailored to your needs and preferences.
What is a Support at Home provider?
A Support at Home provider is an organisation or entity that is approved and accredited by the Australian government to deliver aged care services to individuals who have been assessed as eligible to receive SaH. The responsibilities and services of an SaH provider include developing a care plan; management of funds; monitoring and reviewing care, and compliance with aged care regulations and legislation. At Neighbourhood Care, we have partnered with Trilogy and can provide services for clients with SaH funding.

How do I book an ACAT assessment in WA or QLD?
To arrange an ACAT assessment in WA or QLD, visit the My Aged Care website www.myagedcare.gov.au or call their helpline 1800 200 422. The process includes a home visit, where an assessor will review your care needs and recommend suitable aged care services.
