Support Work

Our aged care support workers provide respectful, reliable assistance to help you stay independent and comfortable in your own home. Whether it’s support with daily routines, personal care, meal preparation or getting out and about in the community, our team is here to support your choices and wellbeing.

We take the time to get to know you, what matters most, what brings you joy and how you’d like to be supported. We work in locally based, self-managed teams, which means flexible, responsive care delivered by people who live in your area. With a focus on dignity, connection and continuity, we’re here to support you in living well, your way.

Allied Health

Our aged care allied health services are designed to support your health, independence and quality of life as you age. We offer physiotherapy, occupational therapy and speech pathology delivered in the comfort of your home, at a pace that suits you. Our therapists focus on practical, personalised goals – whether that’s improving mobility, preventing falls, adapting your home environment or enhancing communication.

We begin with a thorough assessment, followed by a plan that reflects your needs, lifestyle and preferences. Our allied health professionals work closely with your support team, GP and family to ensure care is well-coordinated and holistic. You’ll see the same familiar faces and receive support that’s grounded in understanding, professionalism and trust.

Nursing

Our aged care nursing services offer professional, compassionate clinical support to help you stay safe and well at home. Whether you need help with wound care, medication management, chronic condition monitoring or continence support, our nurses provide personalised care with dignity and respect. At Neighbourhood Care, we understand that feeling safe and cared for starts with familiar faces and genuine relationships.

That’s why our locally based, self-managed teams take the time to get to know you and your preferences. Working closely with your GP, family and support workers, our nurses ensure your care is well-coordinated and tailored to your needs. With a focus on comfort, continuity and trust, we help you maintain your health while living the life you choose.

Care Coordination

Our care coordination services are here to help you navigate aged care with ease and confidence. From organising services to coordinating with your GP and support team, we ensure your care is seamless, personalised and responsive to your changing needs. At Neighbourhood Care, we take a hands-on, human approach – listening to your goals, respecting your choices and helping you access the right support at the right time.

You’ll work with a dedicated care coordinator who knows your story, advocates on your behalf and keeps everything connected. Because our teams are small and community-based, we can respond quickly and work closely with your family and network. Our aim is to reduce complexity and help you feel supported every step of the way.

Frequently Asked Questions

Support at Home (SaH) is a government-funded programme designed to provide older Australians with the necessary support and assistance to help them remain living independently in their own homes for as long as possible. It offers funding for tailored home help services that can include a range of supports. In partnership with Trilogy Care, Neighbourhood Care offers a wide range of in-home aged care services, including help with personal care, household tasks, nursing, allied health and staying connected to your community. All support is tailored to your needs and preferences.

A Support at Home provider is an organisation or entity that is approved and accredited by the Australian government to deliver aged care services to individuals who have been assessed as eligible to receive SaH. The responsibilities and services of an SaH provider include developing a care plan; management of funds; monitoring and reviewing care, and compliance with aged care regulations and legislation. At Neighbourhood Care, we have partnered with Trilogy and can provide services for clients with SaH funding.
