Their commitment extends beyond helping people find new homes as they work to expand their networks and create opportunity to educate the sector on how we can change to better support those in need.

We use the Buurtzorg Layered Care Model, known in the Netherlands as the Onion Model, to understand and strengthen the full circle of support in your life.
This model helps us see not just the care we provide, but also the relationships, resources and connections that are already part of your world.
1. First Meeting
From our very first meeting, we take time to learn about your personal values and existing networks. Together, we begin planning support that truly reflects your life, priorities and goals. By involving you and those around you, we can create a plan that builds on what’s already working and fills the gaps where needed.
2. Person – The Heart of the Model
We start with you. We take into account your values, preferences and what matters most. This might mean maintaining control over your decisions, safeguarding your quality of life or nurturing meaningful, trusting relationships.
3. Informal Networks
Next, we explore your informal supports: family, friends, neighbours and others in your community who play a role in your well-being.
4. The Neighbourhood Care Team
We then introduce our dedicated team of healthcare professionals – the people who will provide consistent, personalised support and work alongside you every step of the way. Our teams are multidisciplinary and made up of a mix of support workers, nurses and allied health therapists, who work with clients across My Aged Care, NDIS, ICWA, Medicare and Ronald McDonald.
5. Formal Networks
Finally, we consider your formal supports, such as your GP, pharmacist, or other service coordinators, ensuring everyone works together for your benefit. This layered approach means your care plan is truly holistic and connects the people and resources that matter most, so you can live the life you choose.
