Description

Neighbourhood Care is growing. Therefore, we are looking for an experienced (specialist) support coordinator in the Northam Shire. It’s a flexible job where every day is different. 

Some of your daily tasks include:

  • Helping participants understand their NDIS plan
  • Working with participants to plan and coordinate their supports
  • Connecting participants with supports and services
  • Establishing and maintaining supports
  • Setting up service agreements and service bookings
  • Coordinating with participants, their support workers and their family members to help participants exercise choice of their supports
  • Supporting participants in finding ways to be more independent

Requirements

  • Minimum 2 years of demonstrated experience as a (specialist) support coordinator with the NDIS
  • Resilient and positive outlook
  • NDIS Worker Screening Check
  • Australian driver’s licence
  • Full Australian working rights
